The City has established procedures for processing claims against the City. Claims must be submitted in writing, on the proper form, and filed with the City Clerk's office, located at 2355 Trane Road, Bullhead City, AZ 86442 .
The fillable Notice Of Claim form (below) must be completely filled out. Three (3) estimates and/or supporting documentation is required to be submitted with claims. Claim forms that do not have all required information may be rejected.
The Risk Management Division will review the claim for settlement. Follow-up will be conducted through the Risk Management office or Southwest Risk Services.
